It cannot be displayed on the checkout screen.
“CC delivery date and time specification” does not support checkout extensions.

Adding to the checkout screen is only available to customers on Shopify’s Plus Plan or above, so you will need to change to the Plus plan and select an app that supports the checkout extension.
